Consumer Health Data Privacy Policy

This policy applies to "consumer health data" as defined in Washington's My Health My Data Act and in similar laws in Nevada and Connecticut. It is provided by 4etverg LLC, publisher of Quitaro, 1209 Mountain Road Pl NE, Ste R, Albuquerque, NM 87110-7825, United States.

Consumer health data we collect, and why

  • In the app: smoking history, quit and slip dates, check-ins, mood and craving logs. These are stored only on your device, to provide the app's features, and we do not receive them.
  • Early-access sign-up: your email address, which may show an interest in quitting smoking. We use it to send the one launch message you asked for.
  • Purchase records from Google Play (order number, product, price, date, and the country, region, state or postal code of purchase), which may show that someone bought a stop-smoking app. We use them for accounting, tax, calculating our charitable donation, refunds and support.
  • Support emails: whatever you choose to write to us. We use it to answer you.

We collect this data directly from you and, for purchase records, from Google Play.

Sharing

We share consumer health data only with service providers that process it on our behalf: our email service provider (sign-ups and support email) and our website host. Google receives purchase information from you directly when you pay. We do not sell consumer health data. We do not share it with affiliates or any other third party, and we do not use geofencing.

Your rights

You can ask us to:

  • confirm whether we collect, share or sell your consumer health data, and give you a copy of it, along with a list of the third parties and affiliates it has been shared with and their contact information;
  • delete your consumer health data. We will also tell our service providers to delete it;
  • stop processing data you consented to, by withdrawing your consent.

To make a request, email support@4etverg.com with the subject "Consumer health data request". We may ask for information that lets us find your data, such as the email address you signed up with or your order number. We will respond within 45 days. If reasonably necessary, we may extend that once by 45 more days, and we will tell you if we do.

Appeals. If we decline your request, you can appeal by replying with the subject "Appeal". We will respond in writing within 45 days. If your appeal is denied, you can contact the Washington State Attorney General (https://www.atg.wa.gov/file-complaint), or the Attorney General of the state where you live.
